| *Please remember that you must cancel your membership by completing a cancellation form at the Cooley Center or the 9th Floor Fitness Center at the SPH building. You may NOT cancel via email, phone or fax. *In order to sign up for a Cooley Center membership, you must visit one of our centers and fill out the membership application. You may also access an application to complete and sign by clicking on the link under the "Download a Membership Application" heading above for a printable application. |
| Faculty/Staff Eligibility All those employed within the JHMI community are eligible including employees of JHH, JHU, KKI, Broadway Services and contractors issued a JHMI ID badge. Employees that are INTRASTAFF are not eligible for payroll deduction. INSTRASTAFF employees must pay in full for Cooley membership. PAYMENT OPTIONS We accept cash or check with the pay-in-full amount made payable to JHU. OPTION #1: Payroll Deduction Initial term of membership is 6 months. The payroll deductions may take one to two payroll cycles to initiate. Likewise, cancellation of payroll deductions may take one to two payroll cycles. Actual membership ceases on the day you cancel at the fitness center. BASIC MEMBERSHIP The Cooley Center includes facility access and use of all courts, cardiovascular equipment, track, weight rooms and locker rooms. *Group exercise and pool access are additional fees. Please ask staff for information). GOLD MEMBERSHIP Gold membership includes all the basic member rights with the added value of unlimited group exercise classes*. *Group exercise classes have a drop-in rate of $4 per class if you do not have the gold membership package. OPTION #2: Pay In Full You can pay in full for 6 months or a full year. The price is discounted by paying in full, in comparison to payroll deductions. SPOUSE AND FAMILY MEMBERSHIPS Memberships are extended to spouses and dependent children only. Proof of residency is required and children must be between the ages of 14-18. Family member fees and terms will be the same as the JHMI staff member’s fee. No one under the age of 14 is permitted in the facility. We offer short-term memberships of one or three month terms (pay in full option only). Your ID badge must reflect your status. SHORT-TERM MEMBERSHIP FEES SPECIAL CASES: Hospital Patients/Relatives Hospital patients and relatives may use the Cooley Center following these guidelines: Patients must supply letterhead documentation from the attending physician’s office. The letter serves as a physician’s referral that states he/she may exercise in an unlimited fashion, without supervision and does not pose a threat to other member’s safety. Family members need a similar letter that verifies their status. $5/day - $15/week - $30/month |

| STUDENT MEMBERSHIPS Subsidized Membership (not required to pay membership fees): Full-time degree candidates of the School of Medicine Full-time degree candidates/students of the Bloomberg School of Public Health All subsidized memberships run from September 1st-August 31st and should be renewed each school year – regardless of when membership was activated. non-subsidized membership (required to pay membership fees): Part-time students/Non-Degree students Homewood, Nursing, Radiology Any other students not listed in the subsidized membership requirements will be considered non-subsidized PAYMENT OPTIONS: We accept cash or check with the pay-in-full amount. Please make checks payable to JHU. Student Membership Fees *2-Member denotes spouse or dependant child. Special situations may apply-- see Staff for clarification. *Dependent children must be between the ages of 14-18. No one under 14 is admitted into the facility – no exceptions! **Family is same as 2-Member with a limit of up to 4 members per family. |
